JOB SUMMARY
Assume responsibility for coordinating nursing personnel to assure optimal delivery of nursing care to residents on a shift, implementing administrative responsibilities, activities, policies, and procedures for the skilled nursing facility while working within their scope of practice and licensure.
ESSENTIAL DUTIES AND RESPONSIBILITIES
STANDARD I: The Supervisor will plan appropriately to meet the needs of the facility.
- Assume responsibility for the entire facility in the absence of the Administrator and Director of Nursing.
- Assess residents’ physical, emotional, and spiritual status to determine nursing care needs.
- Monitor implementation of appropriate nursing interventions in accordance with administrative and departmental policies and procedures.
- Plan staffing patterns in conjunction (when possible) with the Clinical Coordinators to meet resident needs during the shift and make recommendations as needed. Notify assisted living when assisted living employee calls in.
- Confer with the Clinical Coordinators to ensure continuity of care between shifts when possible or necessary.
- Respond to code/emergency calls in Palmer House and Wesley Park.
- May need to take assignment on a unit in an emergency situation in skilled nursing facility.
- Help with admissions and discharges of residents.
STANDARD II: The Supervisor will organize appropriately to meet the needs of the nursing units and Department of Nursing.
- Assign staffing according to resident needs during shift to ensure adequate distribution and utilization of appropriate personnel.
- Take ill/absent calls from nursing employees. Record appropriately to ensure proper documentation and follow-up in call in book.
- Notify assisted living of any assisted living call ins.
- Coordinate unit activities, including:
- Delivery of resident care through supervision of work processes.
- Orientation
- Evaluation of personnel performance as needed.
STANDARD III: The Supervisor will provide direction and leadership to nursing personnel under his/her immediate supervision.
- Give clear directives and provide appropriate feedback.
Utilize own expertise in developing and educating staff members. - Assume leadership role in response to emergency/fire codes or other crisis situations.
- Identify potential safety hazards within the work environment and initiate corrective action. Monitor compliance with safety precautions.
- Demonstrate effective problem solving.

- Assess residents for LPN’s.
- Assure appropriate follow-through (Fall evals, skin assessments; notes of interview.)
- Help determine appropriate intervention.

STANDARD IV: The Supervisor will assist in ensuring a high quality of nursing care services.

STANDARD V: The Supervisor will participate actively in programs which will enhance his/her professional growth as well as contributing to that of other staff members.

- Maintain CPR certification.

- Demonstrate awareness of legal consequences of nursing action.

STANDARD VI: The Supervisor will maintain acceptable work standards.
- Maintain appearance in compliance with The Village at Manor Park nursing standards.
- Demonstrate respect for persons and property.
- Obtain all mandatory in-house education, including skills review and competency testing.
- Maintain mandatory 12 hours of continuing education units (CEU’s) annually, as required per professional staff in service requirement policy.
- Observe time limits in respect to break, lunch/supper.

SUPERVISORY RESPONSIBILITIES
Supervises 15 to 30 employees in the Nursing Department. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. The supervisor is responsible to notify administrator and D.O.N. and social services via group text message of any and all incidents, accidents, allegations and resident concerns.

EDUCATION and/or EXPERIENCE
Associate’s degree (A. A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience. RN license is required.
LANGUAGE SKILLS
Ability to read and write in English. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Ability to use computer and cell phones.
MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry. Add, subtract, multiply divide and use decimals.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
Licensed R.N. in State of Wisconsin
OTHER SKILLS AND ABILITIES
Must be able to handle stress when dealing with residents, families, and staff.
Use hands for difficult, manipulative tasks (like IV insertion).
OTHER QUALIFICATIONS
3-5 years geriatric experience preferred
Must meet all qualifications for R.N. staff nurse.
